You do not have to - but if the extras are damaged or stolen they will not pay you because they have no record that they are on the car.

For example, lets say someone steals your $3000- rims and tires. Stock rims and tires (used/refurb) are $500- and you have a $250- deductable, you will get $250.

No, it is probably better that you do not tell them. Because they will try to blame any damage to the vehicle on the mods. l know some insurance companies wo not cover your transmission or drive train if you put lowering springs on your car.
